Patient Forms

If you are a new patient to our office, please visit our patient forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. Payment Options

For your convenience we accept Amex, Visa, MasterCard and Discover. We also accept payment through HSA / Flex spending accounts. In order to deliver the finest care at the most reasonable cost to our patients, payment is required at the time service is rendered unless other arrangements have been made in advance. If you have questions regarding your account, please don’t hesitate to contact us at (919)819-9289. 

You are fully responsible for all fees charged by Robert Sopko Endodontics regardless of your insurance coverage.

We will send you a monthly statement of your account. Please call our office if your statement does not reflect your insurance payment within a four to six week timeframe. Any remaining balance is your responsibility. Your prompt remittance is appreciated
